Lisa aims to develop a digital communication tool (mobile app) to support and optimize compassionate, person-centered care for older adults (residents) in long-term care homes. The digital communication tool builds on previous research that tested a paper copy of the tool to support resident and family caregivers in collaborative care planning. A digital communication tool will be developed with input from digital technology experts, healthcare providers, residents and family caregivers through a series of workshop discussions, and a survey for feedback on the final tool design (prototype). The tool can empower collaborative decision-making around areas such as care transitions or changes in care needs, while ensuring compassion is at the forefront of these discussions. Lisa’s work will ensure that human interactions and person-centered care are at the center of long-term care digital innovation.
