Canadians are living longer but our health-care system is not ready. Older persons often develop chronic diseases and problems like dementia and falls. As a result, they have frequent hospitalizations and worsening disability. These outcomes could be prevented if care providers cooperated to help older adults stay well. Melissa’s project brings together older adults and health- and social-care providers to understand how to use a digital health tool for better and more compassionate care.
